Their patients are expected to seek care within their FHO group and should not attend outside walk-in clinics for care that can be provided by their FHO physicians. Please note that Dr. Mousavi, Dr. Shafiee, and Dr. Ghomeshi are no longer part of this cross-coverage arrangement and therefore do not provide visits for patients rostered with Dr. Madarek, Dr. Nari Rad, or Dr. Talaei. Patients of Dr. Mousavi, Dr. Shafiee, and Dr. Ghomeshi may seek care from other physicians or walk-in clinics when needed. Dr. Mousavi and Dr. Shafiee will, however, continue to see their own enrolled patients for urgent medical needs or emergencies.

Important About Booking Appointments
Registered patients should use the registered patient booking portal. New patients wishing to register with a PreciseCare FHO physician should use the new patient booking link.
Patients may also book an appointment by calling the clinic and speaking directly with the front desk. If the front desk is unavailable, please leave a callback request on the answering machine, and our staff will return your call. Please kindly avoid sending emails to request or book appointments. Patients who can not attend their appointment should cancel their appointment. Patients who are unable to attend their scheduled appointment are kindly asked to cancel in advance so that the appointment may be offered to another patient. Patients who miss an appointment without providing prior notice may be required to pay an administrative fee before rebooking another appointment.
